UNSUNG HERO NOMINATION
The search for sport's Unsung Hero is taking place across the United Kingdom.
The Wales winner will be unveiled at the BBC Wales Sports Personality of the Year show on Sunday, 2 December at the Celtic Manor Resort, Newport.
He or she will then be invited to the Sports Personality of the Year show in London on Sunday 9 December along with the winners from other areas, where an overall winner will be announced.
Northern Ireland, Scotland, East, East Midlands, London, North East & Cumbria, North West, South, South East, South West, West, West Midlands, Yorkshire, East Yorkshire & Lincolnshire will also nominate their unsung heroes.
Last year Mandy Keating of Georgetown Boys & Girls Football Club, near Merthyr Tydfil, won the Wales award.
HOW TO MAKE A NOMINATION
To enter, you will need to complete all sections of the nomination form (note: users reading this article via our mobile service will need to view this page on a pc to enter online).
Send the completed form before the closing date of Saturday 3 November to:
