Sports Unsung Hero 2009

Do you know someone who helps others take part in sport within your community? Someone who goes that extra mile to make a difference to sport?

If so, nominate them for the BBC Sports Unsung Hero 2009 award. Winners will be named for each of the BBC's 15 Nations and Regions, including Scotland.

We are looking for an individual who, on a voluntary basis, makes a real difference to sport.

This person will give their time and talents to enable others to play sport.

This may be in helping an individual, team or teams.

• This sport can be at any level.

• They must be someone who gains no financial reward for their time.

• They must be aged 16 years or over.

A regional judging panel, made up of a cross-section of BBC Scotland sports staff and other local sports experts, will decide the winner of the BBC Scotland Sports Unsung Hero 2009.

All 15 BBC regional winners will then be invited to this year's BBC Sports Personality of the Year show, broadcast live from the Sheffield Arena on Sunday, 13 December on BBC One, where an overall winner will be announced.

Previous overall winners

Margaret Simons was the 2007 winner. She has run Bardwell Football Club in Oxfordshire since founding it in 1964.

Ben Geyser is the driving force behind Dorchester Boxing Club. He won the award in 2008.

Alex Sutherland - Inverness Swimming Club

Last year's BBC Scotland winner, Alex, used to coach between 80 and 90 people every day of the week.

He has been a volunteer for 60 years, only taking a break during the Second World War.

To enter, you must ensure all the sections are filled in. Any nomination that doesn't include details of the nominee cannot be accepted.

Your nominee must be informed and have agreed to being entered, nominees may be asked to appear on BBC TV and/or radio.

The judges' decision is final and no correspondence will be entered into regarding their decision.

The competition complies with the BBC's new Code of Conduct on Competitions and Voting which is published on the BBC's website.
