Regional winners will be invited to the star-studded Sports Review of the Year show at Television Centre in London.
The overall Unsung Hero wiill be announced as part of the programme's 50th anniversary, and join other prize winners including the Sports Personality of the Year.
They must be aged 16 years or over.
The search for Sport's Unsung Hero is taking place across the United Kingdom.
It will see winners initially named in the following BBC areas: Northern Ireland, Scotland, Wales, East, East Midlands, London, North East & Cumbria, North West, South, South East, South West, West, West Midlands,
Yorkshire & Lincolnshire.
All 14 regional winners will be invited to the Sports Review of the Year show on BBC One on Sunday 14 December 2003 where an overall winner will be announced.

You will need to complete all sections of the nomination form.

Your nominee must be informed and have agreed to being entered. Winners will be announced on or before Sunday 14 December 2003.
Nominees may be asked to appear on BBC TV and/or radio. The judges' decision is final and no correspondence will be entered into regarding their decision.
