Leicester Tigers coach Marcelo Loffreda said his side's performance in the defeat away against London Irish was littered with errors.
A Tigers win would have taken them top of the Premiership but they lost 22-13 despite Ayoola Erinle's early try.
"Most things went wrong. We didn't have good ball and when we did, we didn't keep it," he told BBC Radio Leicester.
"We lost posession very easily, there was a loss of concentration in some basic things that we should deal with."
He added: "There were some times where we should score and we didn't and you pay a high price for that and in the second half we didn't play in the right areas."
