England battled to a comprehensive win over a physical Samoa side after a late flurry of points at Twickenham.
Tom Voyce's try, together with 11 points from Charlie Hodgson's boot, gave England a 16-3 half-time lead.
A tired Samoa struggled after the break with Voyce's second score paving the way for tries from Hodgson, Harry Ellis and debutant Tom Varndell.
Lewis Moody and Alesana Tuilagi were both sent off late on as a simmering game boiled over.
Some questionable Samoa tackling saw tempers become increasingly frayed and when Tuilagi took Mark Cueto out in mid-air five minutes from the end, players from both sides took the law into their own hands.
Tuilagi - already warned for a high tackle - sent Cueto crashing to the ground and the England winger took exception, sparking a mass punch-up.
Tuilagi's tackle on Cueto in the air sparked the late punch-up
Moody aimed several punches at his Leicester team-mate Tuilagi, becoming the first England player to be sent off at Twickenham.
But Samoa captain Semo Sititi was fortunate not to have followed both players off the pitch after landing several blows on Moody.
England's five tries belied a continued struggle for fluency behind the scrum despite Hodgson's prompting.
Census Johnstone was penalised for boring in at the scrum and Hodgson kicked England into a 3-0 lead on eight minutes.
Louis Deacon carelessly knocked-on straight from the kick-off to hand the initiative back to Samoa. Matt Stevens conceded a penalty in front of the posts for Vili to level the scores.
Samoa's indiscipline at the breakdown meant the scores did not stay level for long as hands in the ruck led to a penalty which Hodgson stroked over from half-way.
With the action regularly punctuated by referee Mark Lawrence's whistle, another penalty came as England's Harry Ellis was harshly adjudged to have offended in clearing out a ruck.
But the kick proved to be just outside of Vili's range as his effort fell short of the bar.
Neither side could put more than a handful of phases together, and it was no surprise when England's first try came directly from a Samoa mistake.
Running a penalty from deep in their own half, Samoa failed to look after possession and Moody was on to the loose ball in a flash.
Swift close-range passing from Ellis and Mike Tindall released Voyce to cross for his first international try.
England were soon battering the Samoa line again with Hodgson showing surprising strength to go just short. Mark Cueto followed up with another effort but the ball squirmed loose.
England hooker Steve Thompson goes on the rampage
Eliota Fuimaono-Sapolu then provided the stand-out moment of the first half, collecting an awful pass before two breathtaking sidesteps broke the line.
He sprinted into the wide open space but with support left and right, chose to kick the ball allowing Lewsey to defuse the danger.
Another Hodgson penalty put England 15 points clear at half-time as the Twickenham crowd waited for a new-look home side to spark.
They were still waiting until some 12 minutes into the second half as a series of 5m scrums - during which captain Martin Corry was held up over the line - when Voyce crossed for his second try.
Hodgson delayed his pass well and Voyce timed his run to perfection to sweep under the posts without a hand being laid on him.
A yellow card for Justin Va'a aided England's cause and soon after Hodgson - England's main creative spark - dived over in the corner after Ellis scooped the ball along the ground.
Samoa went down to 13 men on 67 minutes when Vili's dangerously high tackle on Cueto was pulled up.
As the Samoa defence tired, quick hands from England's forwards opened up the space for Ellis to show his pace off the mark and go over for his side's fourth try.
The red cards raised the temperature of the game, and there was still time for a debut try for substitute Varndell as he pounced on Lome Faatau's mistake to round off the win.
