England sevens star Vickerman scored the winning try for Leeds
Rob Vickerman's try in the seventh minute of injury time broke Nottingham hearts and sent Leeds Carnegie into the EDF National Trophy final.
Nottingham had led from as early as the 12th minute when Tim Taylor's try was converted by David Jackson, who then exchanged penalties with Jason Strange.
Alberto Di Bernado kicked the visitors to within four points at 13-9.
Jackson agonisingly hit a post with another kick and was made to pay when Vickerman touched down in the corner.
Leeds will now face Moseley in the final at Twickenham after they ran out 32-24 winners over Exeter Chiefs on Saturday.
Nottingham return to action in the league at Rotherham Titans on Saturday as they chase a top-four finish in National One.
