Everton required penalties to overcome Bristol City after being held to a 2-2 draw at Ashton Gate.
David Moyes' team seemed set fair at 2-0 up after Duncan Ferguson scored from a penalty and Luke Chadwick netted.
But City worked their way back level with an Alan Stubbs own goal and an effort from Leroy Lita and that stayed the score after 120 minutes.
Then Lee Carsley stepped forward to slot home the vital spot-kick and send the Toffees into the third round.
