Brett Ormerod scored the only goal as Preston defeated Sunderland for the third time this season to reach the fourth round of the FA Cup.
Ormerod struck after collecting a cross from Callum Davidson and shooting past keeper Darren Ward and defender Jonny Evans, who was stood on the line.
Sunderland lost Liam Miller to a second yellow card before the break when he brought down David Nugent.
Nugent later went close for Preston, as did David Connolly for Sunderland.
Preston manager Paul Simpson:
"It was hard work and we made it hard work. It was one of those games when a team goes down to 10 men, they raised all their levels and unfortunately we dropped ours a little bit.
"But in the FA Cup the only thing you concern yourself with is you make sure you get into the next round and I'm delighted to say we've done it.
"They pushed us all the way and they've probably gone away a bit gutted that they haven't got anything out of it."
Sunderland manager Roy Keane:
"We're disappointed and we felt we deserved to get something out of the game but it wasn't to be.
"Considering we only had 10 men we had lots of possession, and we had one or two half chances towards the end.
"It can be a cruel game sometimes and it's been a cruel day for us today because I thought the players were outstanding.
"We made one mistake, playing the offside for the goal, and we got punished for it. You've got to give Preston credit because they didn't have too many chances but they took it."
