Forest dominated for long periods and went ahead when Gary Holt started and ended a fine passing move by slotting in his first goal since November 2005.
Kris Commons had already hit a post and Forest doubled their lead when James Perch headed in Grant Holt's cross.
Substitute Nathan Tyson wrapped it up by slamming in a last-gasp penalty after Shane Duff fouled Luke Chambers.
