Spinners Ian Salisbury and Nayan Doshi shared six wickets for Surrey against Worcestershire at the The Oval.
Resuming on 55-0 on the third day, the visitors reached 314-6 after a half-centuries from openers Phil Jaques (61) and Stephen Moore (75).
Worcestershire showed resilience as Steven Davies (40) and Kabir Ali (14) shared an unbroken stand of 41 for the seventh wicket.
But they were still 38 runs short of avoiding the follow-on.
Left-armer Doshi's four wickets were his best return since 2004.
