Surrey's Nayan Doshi grabbed five wickets on day two at Old Trafford as Lancashire lost ground they could ill afford to lose.
The hosts want a win to stay hopeful of remaining in the top flight, but they conceded a first-innings deficit of 91 when bowled out for 278.
Doshi returned 5-125 and keeper Jon Batty had a hand in six dismissals, while Glen Chapple top-scored with 67.
The visitors extended their advantage by reaching 8-0 at stumps.
They were earlier bowled out for 369 in their first innings after resuming play on 346-7.
