Derbyshire captain Dominic Cork took three quick wickets as Worcestershire made a stuttering start in reply at the County Ground.
Cork finished with 3-32 off 15 overs, while Graeme Welch chipped in with a brace.
England star Vikram Solanki was Worcestershire's highest scorer with 28 but Gareth Batty (23 not out) and Steve Rhodes (26 not out) were well set at stumps.
For Worcestershire, Australian Matthew Mason took 5-43 off 14 overs as Derbyshire's batsmen struggled to get settled.
Mohammad Kaif top-scored for Derbyshire with 31 as South Africans Nantie Hayward and Andrew Hall took two wickets each.
