Coroner Andrew Haigh says he was unaware of failure

The South Staffordshire Coroner, Andrew Haigh, has been giving evidence to the public inquiry into why concerns weren't raised about the problems there.

Dr Haigh insists that he wasn't aware of systemic failures at Stafford Hospital because he was dealing with "individual cases" and that "coroners really become concerned about a group of cases if it is absolutely blatant" and he gave the example of C-Difficile.

As a result he didn't realise there was a problem until 2009 when the Healthcare Commission Report came out.
