Have you ever been cyberbullied?

Newsround are looking for real life stories to include in a special programme about online safety.

Here's how you could get involved with our online safety programme.

Have you ever been a victim of cyberbullying?

Has someone ever nicked your password, or passed round your photos or your phone number without your permission?

Or has someone ever set up a fake site pretending to be you, or to be nasty to you?

If so, Newsround wants to hear from you.

If you would like to share your experiences please get permission from your parent or guardian as we would like to talk to you about what happened.

We wouldn't need to film you, so don't worry if you feel a bit embarrassed about what happened.

If we included your story we would be using actors and animation to tell your experiences, so we would never reveal your identity on the programme.

If we follow up your story, we'll get in touch with your parents/guardians by telephone - so make sure you include their home number in the form below.

We won't do anything with your information until we get permission from your parents.

Some of the questions we ask might be quite difficult to answer, or make you feel upset.

Online safety is a serious subject but help is available. See the links below if you are worried and want some advice:

Once this programme is finished, we won't keep any of your information and details on file and we won't pass them on to anyone else.
