A&E emergency waits
A measure of whether patients were left waiting more than twelve hours for admission via A&E.

Cancelled operations: Patients not readmitted
A measure of the number of patients not readmitted within 28 days of the cancellation of their operation.

Hospital cleanliness
Whole trust score of hospital cleanliness, worked out after Patient Environment Action Team (PEAT) visits.

Inpatients waiting too long
Number of patients who were waiting more than 15 months throughout the year, or more than 12 months at end of March 2003, for an inpatient admission.

Outpatients waiting too long
A measure of the number of patients who were waiting more than 26 weeks throughout the year, or more than 21 weeks at end of March 2003, for an outpatient appointment.

Two-week cancer waits
Percentage of patients seen within two weeks of urgent GP referral for suspected cancer to outpatient appointment with a specialist.

Deaths within 30 days of heart bypass
Deaths within 30 days of surgery for heart bypass operation, (which includes deaths in hospital and after discharge). The number is the rate per 100,000 operations, rather than the number of deaths.

Deaths within 30 days of other operations
Deaths within 30 days of a set of selected surgical procedures (which includes deaths in hospital and after discharge). The number is the rate per 100,000 operations, rather than the number of deaths.

Emergency readmission
Emergency readmission to hospital following discharge, as a percentage of live discharges

Emergency readmission after hip replacement
Emergency readmissions to hospital within 28 days of discharge following treatment for a fractured hip, as a percentage of live hip fracture discharges.

Emergency readmission after stroke
Emergency readmissions to hospital within 28 days of discharge following a stroke, as a percentage of live stroke discharges.

Drug resistant bacteria infections
A score given to each hospital for improvements to their incidence of Methicillin Resistant Staphylococcus Aureus (MRSA) bacteraemia. The score is from A to E, where A is the best and E is the worst.

Thrombolysis treatment: How quickly?
Many heart attack patients could benefit if given clot-busting thrombolysis treatment quickly. The figure shows the percentage of eligible patients treated with thrombolysis within 30 minutes of hospital arrival.

Bed-blocking: Delayed care transfers
Percentage of patients whose transfer of care from hospital was delayed.

Nine-month heart operation waits
Percentage of elective patients waiting less than 9 months for a "revascularisation" operation, such as a heart bypass.

Patient survey: Access and Waiting
Score taken from patient feedback about access to treatment and waiting times. The score is from one to five.

Patient survey: Clean, comfortable, friendly?
Score taken from patient feedback about the state of hospital facilities and staff attitudes. The score is from one to five.

Patient Privacy/Dignity
Score giving progress towards government target of eliminating mixed-sex wards. The score is from one to five.

Six-month inpatient waiting
Number of patients who had been waiting less than 6 months for inpatient treatment at quarter end divided by the number of patients on inpatient's waiting list, expressed as a percentage.

Thirteen week outpatient waits
Number of outpatients seen within 13 weeks of GP written referral divided by the total number of outpatients seen following GP written referral, expressed as a percentage.
