An inquiry has begun after a five-year-old girl left a Leeds school unnoticed and walked a mile across the city with bare feet.
She is thought to have been changing for a PE lesson at Kerr Mackie Primary School before wandering off into the Roundhay area of the city last Friday.
She crossed busy roads before reaching her childminder's house, after first trying her own house, which was empty.
Health and safety staff from Education Leeds are leading the investigation.
It is the second time since May they have been called in to review school safety procedures in Leeds.

On 1 May, a mother found her three-year-old son wandering in a street when she arrived at Kippax Ash Primary's nursery school to collect him.
After Tuesday's incident, Kerr Mackie head teacher Jon Farley said: "I deeply regret this incident and am extremely thankful she was found safe and sound.
"I have written to every parent and carer to reassure them that we take the health and safety of our children extremely seriously, and advise them of the immediate action we've taken in light of this incident.
"I immediately began an investigation, assisted by Education Leeds, and will take whatever action necessary to stop this ever happening again," Mr Farley said.
An Education Leeds spokesman said the school gate would now be checked each morning to ensure it was locked.
Mr Farley said: "I have reiterated to all staff the importance of ensuring pupil safety and following strict safety procedures at all times.
"I will continue to keep our families informed of additional measures we introduce and will do everything I can to address their concerns over this or any other matter."
