The law firm hired by Siemens to investigate claims of corruption there says it has discovered violations.
Debevoise & Plimpton examined transactions between 1999 and 2006.
Investigators are looking into whether Siemens employees paid bribes to secure telecoms deals. The company has admitted dubious payments were made.
The internal report did not name any individuals involved and Siemens issued a statement saying it has full confidence in its current management.
"The violations in question reflect not only outright incidents of corruption, but in many cases violations of regulations pertaining to internal controls and the accuracy of documentation," the report said.
Siemens agreed in October to pay a fine of 201m euros ($313m; £159m) to settle some German legal proceedings.
It is still being investigated by authorities in several countries including the US, Nigeria, Norway and Malaysia.
Siemens has said that US authorities are willing to negotiate a settlement but added that it expects talks about it to take several months.
