Inquiry ordered into hospital surgery blunder
Breast cancer was misdiagnosed

The Welsh Health Secretary has ordered an inquiry into a hospital blunder which caused a woman to have unnecessary major surgery.

Lynda Davies was misdiagnosed with breast cancer while a patient at the West Wales general Hospital in Glangwili near Carmarthen.

She subsequently underwent emergency surgery to remove one of her breasts.

Health Secretary Jane Hutt: "Lessons to be learnt"
The Carmarthen and District NHS Trust has admitted that the surgery was unnecessary and a mistake was made.

Welsh Health Secretary Jane Hutt has now ordered an inquiry into the case.

Ms Hutt told the Assembly's health and social services committee that she has asked the trust's chairman, Tom Norris, for a full report of the events which resulted in the unnecessary surgery.

"I have also asked the trust for a note of the action it has taken and any future action proposed to reduce the chances of this kind of error happening again," she said.

"(Assembly) officials will determine if there are any lessons to be learnt from this unfortunate incident for any other hospitals throughout Wales."

Disciplinary action

Ms Hutt has also written to Lynda Davies expressing her concern.

Mrs Davies has called for disciplinary action to be taken against the consultant responsible.

Ms Hutt said she understood the trust had undertaken an internal inquiry at the time of the events and that no disciplinary action was taken against the consultant for what was deemed to be a genuine error.

Mrs Davies is currently pursuing a claim for compensation with the trust.
