What went wrong at polling stations?

Many people were unable to vote in the 2010 general election as some polling stations were overwhelmed by the turnout.

Meanwhile, in Lancashire, a 14-year-old boy managed to cast a ballot after he was accidentally sent a polling card.

The BBC's Phil Lavelle has been looking at how problems like these have cast a shadow over the 2010 general election.
