A prisoner has hanged himself in his cell in Norwich Prison, a Home Office spokesman has confirmed.
Simon Smith, 40, who had been jailed for life for wounding, was discovered by prison staff on Friday afternoon, a Home Office spokesman said.
He confirmed Smith, who was jailed in April 1992, was in a cell in the health care unit.
The spokesman said: "He was being actively monitored for self-harm and suicidal behaviour."
He added: "Staff tried to revive him, but sadly he was declared dead at 1325 BST.
"His family have been informed, and our thoughts are with them at this difficult time."
An investigation into Smith's death was launched by the prison and probation ombudsman Stephen Shaw.
