Faulty equipment and human error caused Hull's Drypool Bridge to lift as cars and pedestrians were crossing, a city council investigation has revealed.
Cars had to reverse and one man leapt across the widening gap over the River Hull in Wednesday's incident.
Council engineers say operator error and a failed safety switch allowed barriers to rise at the wrong time.
Safety switches have been replaced and operating procedures reviewed to Health and Safety Executive satisfaction.
