Devon and Cornwall Police are investigating who leaked a report into a pay review fiasco.
Hundreds of staff walked out last April in protest at re-grading proposals which would have left some with pay cuts of up to £8,000.
The force later ditched the re-grading exercise and ordered a report by the Metropolitan Police.
That report, criticising the Devon and Cornwall force, has just been released, two weeks after it was leaked.
The Metropolitan Police said the re-grading exercise failed and described "a series of actions by different individuals, some very minor, which, when taken together, resulted in a major failure".
It went on: "There appeared to be a lack of understanding of the strategic importance and significance of the project by some members of the command team [the force's most senior officers]."
