A new super hospital in Derby will be known as The Royal Derby after approval for the name was given by the government, managers said.
The Royal title will be transferred from the Derbyshire Royal Infirmary to the new building on Uttoxeter New Road.
The hospital will be one of the largest teaching hospitals in the UK with 1,159 beds and 35 operating theatres.
Derby Hospitals NHS Foundation Trust said the name had been confirmed by the Ministry of Justice.
A trust spokesman said: "We are delighted that our request to retain the Royal charter has been approved."

"We are extremely proud of the Royal charter that has been associated with the well-known Derbyshire Royal Infirmary for many years and this historical link will now continue as we look to the future with Derby's new hospital," the trust's project director Brian Ibell said.
It is expected the new name will be used from early 2009 when the new main entrance at the hospital is opened.
The Derbyshire General Infirmary was granted its Royal charter in May 1891 following Queen Victoria's visit in May 1891
The scheme has been funded by the private sector, with the NHS leasing the facility for 35 years.
