Shaun Stevens was not wearing a safety harness when he fell

An inquest has opened into the death of a man who died after falling from scaffolding two years ago.

Shaun Stevens, 41, was working for Flooks Scaffolding on at site in Kingswood, near Bristol, when he fell up to 20ft (6m).

He died in hospital 12 days later as a result of his injuries.

The coroner's court heard Mr Stevens was issued with safety equipment, including a harness, but was not wearing it when he fell.
