A nurse who mistakenly gave a premature baby a day's worth of morphine in 11 minutes has been banned from working with children for a year.

Eva Serenio, 49, administered the drug at Bradford Royal Infirmary in September 2004.

The Philippines-trained nurse admitted failing to perform an adequate checking procedure.

A disciplinary hearing of the Nursing and Midwifery Council has also ordered her to be supervised.

These conditions of practice, which include a ban on working in neo-natal care or paediatrics, will be reviewed after a year.

Serious charges

Ms Serenio had also admitted a second charge of failing to perform an adequate checking procedure when handing over eye drops and folic acid to the family of another baby.

Giving her ruling, the chairman of the disciplinary panel, Kathleen McLean, said: "These are serious charges, one of these incidents involved the maladministration of a controlled drug which could have had very serious consequences."

The baby who was prescribed too much morphine later died, but a coroner ruled the overdose did not cause his death.

Kathleen McLean added: "The panel fully acknowledges that in respect of both charges serious system failures and errors by colleagues contributed to your own errors.

However, as a registered nurse accountable for administering these drugs you failed to carry out proper checking procedures and failed to safeguard your patients."
