Boy's death investigated by trust

The circumstances surrounding the death of a 12-year-old boy has prompted an investigation by a primary care trust in Berkshire.

Daniel Farr, from Tilehurst, Reading, was pronounced dead on arrival at the Royal Berkshire Hospital on Sunday.

The BBC understands his family made calls to an out-of-hours doctor service prior to his death.

A post-mortem examination is scheduled to be carried out to find out how Daniel died.

'Delightful student'

In a statement, Berkshire West PCT said: "We are all saddened by this very tragic event.

"A thorough investigation will of course take place into all the circumstances of the case."

Sally Elliott, head of Year 8 at Prospect School in Tilehurst where Daniel was a pupil, said: "Staff and students were totally devastated to hear the tragic news about Daniel.

"He was a lovely young man, and a delightful student.

"He was quiet, hard-working and had a lot of friends, who are all missing him terribly."

The school has been offering support to pupils and said it will be working with them on a suitable way of commemorating his life.
