Agencies 'lacked urgency' over Baby P

A second serious case review of the Baby Peter case has concluded that his death "could and should have been prevented".

The new independent chairman of Haringey Council's local safeguarding children board, Graham Badman, says the agencies that were supposed to protect him lacked urgency and thoroughness.
