Trust chairman on who was to blame

The chairman of the BBC Trust Sir Michael Lyons has said the primary failure involved in the Russell Brand, Jonathan Ross row was one of editorial judgement.

Sir Michael Lyons said the programme in which the two performers left obscene messages on actor Andrew Sachs's answer phone should "never have been broadcast".

The Trust chairman was talking to the BBC's Media Correspondent Nick Higham.
