Panorama still wants to hear from you if you have been suffering from adverse reactions to Seroxat.

Panorama will then send you a yellow card form which can be filled in and sent back to the programme.

The Panorama yellow card is based on an idea from the Medicines and Healthcare products Regulatory Agency (formerly the Medicines Control Agency). They have a 'Yellow Card' form which allows doctors to log complaints that their patients have with drugs have.

The yellow card idea was also used by Panorama when it carried out the detailed survey, in conjunction with the charity MIND, into the side effects of the drug Seroxat.
