The Sarafiya Bridge over the Tigris River in Baghdad was badly damaged when a truck filled with explosives blew up as morning rush hour was getting under way.
Sections of the bridge collapsed, sending several cars plunging into the river. Divers have been searching for survivors.
Most bridges in Baghdad have checkpoints at either end which are intended to stop such attacks, but this time the bombers got through.
The force of the explosion woke up nearby residents and sent a large cloud of smoke rising into the air.
The bridge is one of the main links across the Tigris in the north of Baghdad.
The bridge was built in the 1920s by the British and damaged by US bombing in 1991.
