Hannah Meredith died two months before her 18th birthday from a heroin overdose in October 2009.

Her aunt Lisa Moore spoke to Victoria Derbyshire on BBC Radio 5 live - and read out excerpts from a letter Hannah, from Llanelli, Carmarthenshire, had written the month before her death in which she vowed to get heroin out of her life for good.

The whole interview is available for seven days at the Victoria Derbyshire programme website.
