Madrid crash report published

Accident investigators in Spain have published their initial report into the Spanair passenger plane crash which claimed 154 lives.

The report concludes that the aircraft's wing flaps were incorrectly positioned and that a warning system failed to alert the pilots. Steve Kingstone reports.
