Hundreds of patients 'wrongly diagnosed'
An "early warning system" to identify medical mistakes and incompetent staff in England is to be set up by the government. It follows the news that a pathologist, who continued working into his 70s, made hundreds of mistakes in testing for cancer and other diseases. The BBC's Fergus Walsh reports.
