South Yorkshire Ambulance Service has lost two of its three star rating
An investigation has been launched into the death of a 73-year-old woman who died after being taken to hospital for a routine check-up.

Jean West suffered head injuries after falling backwards out of a wheelchair outside the Northern General Hospital in Sheffield.

The Health and Safety Executive and police are investigating the incident.

Jeremy Pease, director of operations at South Yorkshire Ambulance Service expressed their sympathy to the family.

He added: "South Yorkshire Ambulance is undertaking a internal investigation."

The service has come under recent criticism from Sheffield coroner Chris Dorries after a woman died following a seven hour wait for an ambulance.

The service has also lost two out of its three stars awarded by the NHS for achievement.
