Inquiry into squat girl's death

Kate Walsh died in a Manchester Road squat
There is to be a case review into the death of a teenage heroin addict from Swindon.

Kate Walsh, 16, was found dead in a derelict squat earlier this month after apparently taking a drugs overdose.

An investigation will be launched to discover what different agencies knew about her addiction before her death.

Kevin Maidment from Wiltshire Police said: "If as part of the process, the agencies can address any shortcomings, they will."

"They will identify what could have been done differently and the lessons to be learnt."

Kate's parents, Debbie and Anthony, said they had approached various agencies, including the police and social services, to help their daughter beat her drug addiction.

They were told nobody could help because she was 16 years old.

Swindon Social Services, Swindon Primary Health Care Trust, the local education authority and police will be involved in the review.
