No action is to be taken against a hospital worker who wrongly dispensed anti-depressants to a nine-year-old girl, causing her to suffer an overdose.
Sherrie Peters, who suffers from a rare thyroid condition known as Grave's disease, fell ill at school as a result of the mistake at Telford's Princess Royal Hospital earlier this month.
An internal investigation found that human error was to blame but concluded there was nothing wrong with the procedures in the pharmaceutical department.
The hospital had already apologised to Sherrie and her family, who are from Aqueduct in Shropshire.
The investigation found the mistake probably happened because the wrong label was put on an anti-depressant, which was then mistaken for the medicine Sherrie should have received.
The hospital said the individual responsible had never made a similar error before and was very upset by what happened.
The findings of the report, which was carried out by the hospital's director of pharmaceutical services, have also been sent to Sherrie's family.
The nine-year-old suffered no lasting ill effects from her ordeal.
