A grandmother travelling with her husband and two grandchildren died in a car collision while on a family day out.
The driver of the second car was left in a critical condition, and six people including the two children needed hospital treatment.
The two cars were travelling on the A40 at Pengawse Hill near Whitland, Carmarthenshire, when the collision occurred.
The dead woman, who has not been named, was a passenger in the front seat of her car.
Firemen had to cut the other driver free before he was airlifted to Morriston Hospital in Swansea.
His injuries were described as serious.
Vital
The woman's husband, grandchildren, and three men travelling in the other car were taken to Withybush and Glangwilli hospitals in Haverfordwest and Carmarthen.
Following the accident, which happened on Saturday afternoon, police said they were keen to trace the driver of a blue Peugeot 309 that may have been in the area at about 1700 BST.
They stressed the car was not in any way involved in causing the accident but added the driver might have seen what happened and have vital information for the inquiry.
The driver or passengers in the car, or anyone with information about the accident, are asked to contact Haverfordwest Road's policing unit on 01437 763355.
