Police probe oxygen death |

| A police investigation has been launched into the incident, one of two at two separate hospitals in Essex. In each case pipes carrying oxygen were blocked while patients were under anaesthetic. The boy, who died on 18 July, was a patient at Broomfield Hospital in Chelmsford. The BBC's John McIntyre reports. |
