Report examines operation blunder

A report on the running of the Carmarthenshire NHS Trust is to be published examining the safety issues behind a kidney operation blunder.

Patient Graham Reeves had a healthy kidney removed instead of his diseased one at Prince Philip Hosptial in Llanelli, west Wales.

The report was commissioned by Welsh Health Minister Jane Hutt to look at the trust's safety procedures and measures taken after the incident.

The Commission for Health Improvement - set up by the govenment to monitor standards in Britain's hospitals - was called in to Prince Philip hospital and the West Wales General Hosptial in Carmarthen after Mr Reeves died.

Although the incident was the catalyst for the report, the commission's remit was to look at safety procedures and systems for monitoring procedures at both hospitals.

It is thought the content of the report will not be particularly critical of the Carmarthenshire NHS Trust.

But there are some concerns that the action plan to improve safety procedures has not been fully implemented, particularly at the West Wales General Hosptial, eight months after the death of Mr Reeves.

The 70-year-old Korean war veteran died on 1 March - five weeks after the bungled operation.

Mr Reeves, a bachelor from Burry Port, Carmarthenshire, died in intensive care at Swansea's Morriston Hospital without regaining consciousness.
